Job summary

Qualifications

• Must meet QIDP certification as set forth by DBHDS.
• Bachelor’s degree in Human Services (e.g. sociology, psychology) required.
• One year of experience with individuals with ID/DD required.
• Virginia Driver’s License with acceptable driving record required; personal auto insurance if used for business.

Responsibilities

• Provide case management services to individuals with ID/DD.
• Assess service needs and develop plans for access to services.
• Liaise with consumer, families, programs and service providers; monitor needs.
• Advocate for consumers and provide education to families and community.
