Job summary

Project FIND seeks a Service Coordinator to work with tenants aged 60+ at the Woodstock Hotel in New York. You will conduct detailed assessments, determine service eligibility, and develop individualized service plans with ongoing counseling and support.

Responsibilities include intake, coordination with multiple departments, documentation, and collaboration with health providers to promote aging in place and access to benefits.

Qualifications

• Bachelor's degree or minimum 2 years relevant experience preferred.
• Licensed LMSW or LCSW preferred.

Responsibilities

• Conduct intake and assessments to determine client needs and eligibility for services.
• Develop and implement service plans with clients; provide ongoing supportive case management and counseling.
• Collaborate with Property Management on housing applications, recertifications, and rent-related matters.
• Coordinate with healthcare providers, home care agencies, and family members to support overall tenant health and safety.
• Provide education to clients and caregivers on aging in place, self-care, and advance directives.
• Facilitate groups on topics such as harm reduction, mindful living, and financial management.
