Job summary

Buckner Home Care, LLC, operating as At Home by Buckner, seeks a Home Care Manager to lead care coordination in Houston. Qualifications

• High school diploma or GED with required experience.
• Must meet HCSSA administrator requirements.
• Eligible to serve as administrator under HHSC regulations.
• Excellent verbal and written communication skills.
• Strong organizational and time-management abilities.
• Ability to work independently with discretion.

Responsibilities

• Coordinate resident and caregiver schedules to ensure quality care.
• Match residents and caregivers to foster strong relationships.
• Monitor scheduling, address coverage gaps and changing needs.
• Develop contingency staffing plans for uninterrupted service.
• Maintain resident and caregiver information in the software system.
• Serve as primary contact for residents, families, and caregivers.
