Job summary

Qualifications

• One year of RN experience in a clinical setting.
• Home health experience preferred.

Responsibilities

• Perform initial and ongoing assessments to identify patient needs and develop a Plan of Care.
• Coordinate care with the interdisciplinary team and family members.
• Provide infection control and universal precautions in patient care.
• Communicate changes to physicians and care team; participate in case conferences.
• Supervise and provide orientation to other nursing staff; on-call responsibilities as assigned.
• Complete timely documentation to ensure compliance with agency policies.
• Travel locally to patients' homes and maintain a valid license and CPR.
