Neighborhood Healthcare’s Riverside County PACE program seeks a Referrals Specialist to coordinate all referrals, authorizations, and follow-ups for participants. The role is onsite, Mon-Fri 8am-4:30pm, with CPT/ICD-10 knowledge and strong communication skills.

You’ll schedule appointments, arrange transport, and maintain EMR records to ensure timely care. The position supports care teams, maintains confidentiality, and engages with families and providers to optimize patient outcomes in a

• Coordinates referrals and scheduling for PACE participants.
• Prepares, processes, and completes referrals and authorizations.
• Arranges transportation for participants to medical appointments.

• Tracks referrals in EMR logs and reports authorization statuses.
• Completes scheduling with CPT codes and follow ups.
• Advocates and discusses referral processes with participants and providers.
