Job summary

One Medical in Kennesaw, GA is hiring a Medical Assistant Health Navigator to support a Senior Health patient panel. You will build trusting relationships, perform in-office and point-of-care tasks, and coordinate care with other providers to improve preventive and chronic care outcomes.

You will partner with an integrated primary care team, handle front-desk duties, and educate patients using motivational interviewing to remove barriers to care. This role is full-time and on-site.

Qualifications

• At least 1 year of experience in a high touch customer service or patient facing role in a healthcare setting.
• Strong written and verbal communication skills.
• Experience working on collaborative, diverse and feedback-driven multi-disciplinary teams.
• Proficiency in computer technology such as typing, navigating the internet and using multiple software systems simultaneously.

Responsibilities

• Capture relevant information about the patient’s health and healthcare experience while rooming patients, taking vitals, and performing screenings.
• Assist with coordinating post-visit care by scheduling appointments and referrals.
• Collaborate with providers to monitor health of a patient panel and preventive care updates.
• Participate in daily operations: answer calls, respond to emails, assist with front desk inquiries, maintain space.
• Provide patient education on chronic disease management and coach patients using motivational interviewing techniques.
• Offer anticipatory preventive guidance to families with children and build trusting relationships.
