Job summary

Abode Care Partners seeks an experienced physician to join a clinical collaborative role under the supervision of the CMO. You will provide day-to-day clinical care to residents in facilities and homebound patients, modeling ethical and evidence-based decision-making for the team.

You will lead medical staff, supervise NPs/PAs, audit orders, and participate in medical directorships as needed. You will help identify growth opportunities and participate in frontline call processes to ensure timely

Qualifications

• Graduate from an accredited school of medicine and is a Doctor of Medicine.
• Two or more years of administrative leadership experience in healthcare.
• Clinical medical experience of seven years or more preferred.
• Board certification in Family Medicine, Internal Medicine or Geriatrics; qualification in Geriatrics preferred.
• Practicing physician licensed and DEA registration in state of employment.

Responsibilities

• Provides high-quality complex and geriatric care to residents and patients across settings in partnership with NP/PA and interprofessional team.
• Keeps high engagement with facility leadership teams, nurses, patients, and families.
• Provides collaborating physician duties for 5–15 NPs/PAs to support their clinical roles.
• Audits up to 10% of practitioner orders/documentation monthly as per state/regulatory parameters.
• Helps identify growth opportunities with facilities in the market/region.
• Participates in frontline call processes for timely response to acute changes and to prevent avoidable hospital transfers.
• Focuses on quality outcomes, care path compliance, and reduction of polypharmacy and costs of care.
• Attends required trainings and meetings to assure best care delivery.
