Abode Care Partners is seeking a Collaborative MD – Primary Care Physician to provide high-quality geriatric and complex care across multiple settings in collaboration with NPs/PAs and an interprofessional team.

The role involves clinical leadership, potential medical directorships, and participation in growth initiatives with senior living facilities. Licensure, board certification, DEA registration, and leadership experience are required.

Qualifications

• Must hold an MD and state medical license.
• Board certification in Family Medicine, Internal Medicine or Geriatrics; geriatrics preferred.
• Active unrestricted licensure to practice in state of employment.
• Current DEA registration.
• Two+ years administrative leadership experience in healthcare.
• Clinical medical experience of seven years or more preferred.

Responsibilities

• Provides high-quality complex and geriatric care to residents and patients across settings in partnership with non-physician practitioners and other interprofessional team members.
• Keeps high engagement with facility leadership teams, nurse practitioners/physician assistants, patients, and families.
• Provides collaborating physician duties for up to 15 nurse practitioners and physician assistants.
• Audits up to 10% of practitioner medical orders and documentation monthly.
• May assume medical directorships for skilled and/or other senior living facilities as needed.
• Helps identify new growth opportunities with skilled facilities and other senior living facilities.
• Participates in frontline/backup call processes for timely response to acute changes of condition.
• Maintains focus on quality outcomes and cost management.
• Attends trainings and meetings as mandated by leadership.
