Job summary

UnitedHealth Group's Optum Home & Community Care is expanding its Senior Community Care program to deliver integrated clinical care across nursing homes, senior housing and home settings.

The Advanced Practice Clinician will conduct comprehensive assessments, document accurately, and collaborate with the care team while supporting patients through short-term sub-acute rehab and end-of-life discussions. Eligible NPs and PAs must hold appropriate certifications and licensure.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Current NCCPA certification and state licensure.
• Active, unrestricted licensure in the state of practice.
• Active and unrestricted driver's license with reliable transportation.
• Current DEA licensure or ability to obtain post-hire.
• Ability to gain a collaborative practice agreement, if applicable.

Responsibilities

• Conduct comprehensive assessments including history, meds reconciliation, vital signs and exam.
• Provide care during short-term sub-acute rehabilitation and advanced illness planning.
• Document accurately with ICD-10 coding.
• Collaborate with interdisciplinary care team.
• Provide counseling and education to patients and caregivers.
