Job summary

Optum Home & Community Care, part of the Optum family, is hiring an Advanced Practice Clinician within the Senior Community Care (SCC) program to provide high-quality care to high-risk members in nursing homes and senior living settings.

You will conduct comprehensive assessments, manage complex conditions, and coordinate with the care team to ensure smooth transitions and patient education. A competitive sign-on bonus is offered for external candidates.

Qualifications

• Active ANCC or AANP certification for NP (Family, Adult, Geriatric, or Adult-Gerontology)
• Active NCCPA certification and state licensure for PA
• Active, unrestricted state licensure without disciplinary actions
• Active and unrestricted driver's license with reliable transportation
• DEA licensure/prescriptive authority or ability to obtain post-hire
• Ability to gain a collaborative practice agreement where required by state law
• Ability to navigate varied environments and perform job duties

Responsibilities

• Conduct comprehensive assessments including medical history, medication reconciliation, vital signs, and physical exam
• Support members during a short term sub-acute rehabilitation episode
• Ensure accurate documentation including ICD-10 conditions
• Communicate and collaborate with the interdisciplinary care team
• Conduct advanced illness and advanced care planning conversations
• Provide patients and caregivers with counseling and education
