Job summary

UnitedHealth Group is seeking a Senior Advanced Practice Clinician to join the Senior Community Care (SCC) team in New Jersey. You will conduct comprehensive assessments, coordinate care across nursing facilities and home settings, and provide patient and caregiver education.

The role emphasizes advanced illness discussions, collaboration with the care team, and thorough documentation within EMR systems. A valid NP/PA certification and state licensure are required.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• For PAs: NCCPA certification and state licensure.
• Active, unrestricted state licensure in the practicing state.
• Active and unrestricted driver's license with reliable transportation.
• Current DEA licensure/prescriptive authority or ability to obtain post-hire.

Responsibilities

• Conduct comprehensive assessments including med history, meds reconciliation, vitals, and physical exams.
• Support members during short-term sub-acute rehabilitation episodes.
• Ensure accurate documentation including ICD-10 codes.
• Collaborate with interdisciplinary care team.
• Lead advanced illness and end-of-life planning conversations.
• Provide counseling and education to patients and caregivers.
