Job summary

Heartline Staffing is hiring a Primary Care Physician for a physician-led, senior-focused primary care organization in the United States. The role centers on full-risk Medicare Advantage with an outcomes-driven, value-based model and a focus on health coaching to improve long-term health outcomes.

You will lead a dedicated care team, manage a panel of up to 450 patients, and participate in physician leadership development within a growth-focused organization.

Qualifications

• MD or DO in Internal Medicine, Family Medicine, or geriatrics required.
• Active state medical license or ability to obtain one required.
• Board eligible in Internal Medicine, Family Medicine, or geriatrics required.
• Current DEA number for Schedule II–V required.
• BLS certification from AHA or Red Cross within 90 days of employment required.
• Fluency in written and spoken English required.
• MS Office (Word, Excel, PowerPoint, Outlook) proficiency required.
• Willingness to travel locally and nationally for training and recruitment required.
• Flexibility to work evenings, weekends, and holidays as needed.
• Board certification preferred.

Responsibilities

• Provide care for older adult patients with acute and chronic illnesses.
• Lead the care team for patients able to come to the office.
• Supervise or collaborate with Advanced Practice Practitioners as applicable.
• Coordinate with hospitalists and transitional-care teams for hospitalized patients.
• Participate in risk, quality management, and clinical meetings.
