Qualifications

• MD or DO in Internal Medicine, Family Medicine, or Geriatrics required.
• Active Virginia medical license or ability to obtain one required.
• Board eligible in Internal Medicine, Family Medicine, or Geriatrics required.
• Current DEA number for Schedule II–V required.
• BLS certification within first 90 days of employment required.
• Fluency in written and verbal English required.
• Proficiency in Microsoft Office Suite required.
• Ability and willingness to travel locally, nationally for initial 2-week training and for recruitment purposes required.
• Ability to function independently and in collaboration with other health care professionals required.
• Board certification in Internal Medicine, Family Medicine, Geriatrics, or similar preferred.
• Maintain board certification through MOC, CME, and/or board exams as required.

Responsibilities

• Independently provide care for older adult patients with acute and chronic illnesses.
• Take full accountability for patient care and outcomes, consulting specialists as needed.
• Engage with the hospitalist whenever a patient is hospitalized.
• Provide assessment, diagnosis, treatment, management, education, health promotion, and care coordination for patients.
• Lead the care team for patients able to come to the office.
• Coordinate with transitional care teams, case managers, and other physicians for hospital, SNF, LTC, or homebound settings.
