Job summary

Centene Corporation in Oklahoma is seeking a field-based Behavioral Health Care Manager who will develop, assess, and facilitate complex care plans for members with mental health and substance use needs, coordinating with providers and families.

Requires a Master’s in Behavioral Health or Social Work or a nursing degree and valid licensure such as LCSW, LMSW, LMFT, LMHC, LPC or RN, plus 2–4 years of related experience.

Qualifications

• Requires a Master's degree in Behavioral Health or Social Work or a Degree in Nursing and 2–4 years of related experience.

Responsibilities

• Evaluates member needs via phone or in-home visits and develops care/service plans.
• Performs telephonic, digital, home, or other site visits to assess needs.
• Develops ongoing care plans for high-acuity members and coordinates providers.
• Coordinates between member, family/caregivers, and care team to ensure access to services.
• Monitors care plans and outcomes; provides recommendations based on needs.
• Facilitates care coordination to ensure timely access to care or services.
• Collects and maintains member information to ensure regulatory compliance.
• Provides education to members and families on procedures, options, and benefits.
• Provides feedback to leadership on opportunities to improve care and quality.
• Performs other duties as assigned and complies with policies.
