Responsibilities

• Conducts thorough medical histories and physical examinations in collaboration with the health care team.
• Performs and interprets routine and specialized diagnostic procedures.
• Prescribes and monitors medications as allowed by license and policy.
• Educates patients about diseases, treatments and side effects; participates in rounds and care coordination.
• Ensures continuity of care and timely discharge planning; supports education and quality initiatives.
