Qualifications

• RN or social worker with interest in care management, case management, or care coordination.

Responsibilities

• Identify patient needs (clinical, psychosocial, financial, cultural) to guide discharge planning.
• Assess health literacy and ensure plan aligns with goals of care and patient preferences.
• Coordinate safe and timely discharges by engaging care teams and arranging post-hospital services.
• Communicate clearly with patients, families, and the care team to develop individualized discharge plans.
• Collaborate with physicians and multidisciplinary staff to incorporate recommendations and test results.
• Serve as liaison between hospitals and post-hospital facilities to ensure smooth transitions.
