Job summary

UPMC is hiring a Discharge Plan Manager to join our Clinical Care Coordination team in Harrisburg for a full-time evening shift. The final candidate will work between 10a-8p or 8a-8:30p, with an every 5th weekend and rotating holiday commitment.

The Discharge Plan Manager coordinates post-discharge needs, engages with the care team, and focuses on an optimal discharge plan with timely utilization of hospital resources to ensure positive patient outcomes outside the inpatient setting.

Responsibilities

• Identify clinical, psychosocial, historical, financial, cultural, and spiritual needs that guide the planning process with the patient to attain optimal outcomes.
• Complete detailed assessment on every patient to establish understanding of medical and social factors, determine capacity for self-care, and identify barriers to discharge.
• Facilitate teams to develop and execute safe and efficient discharges; ensure post-hospital care arrangements are made before discharge.
• Engage in clear communication with patients, caregivers, and the interdisciplinary care team to develop individualized discharge plans.
• Serve as a liaison between hospitals and post-hospital care facilities and coordinating physicians.
• Demonstrate shared accountability in discharge plan development with patient and care team to ensure optimal outcomes.
• Align practice with the mission and values of the organization; maintain compliance with regulatory requirements.
• Advocate for services access and patient safety, well-being, and rights.
• Manage cost of care with focus on safety, quality, risk, and patient satisfaction to support optimal outcomes.
• Embrace innovation and technology to improve collaboration and patient outcomes; document care in patient chart.
• Provide staff orientation and mentoring as appropriate.
