Job summary

Colonial Group is seeking a dedicated Case Manager to assess service needs, develop individualized care plans, and coordinate in-home and community support for older adults and people with disabilities.

You will work with consumers, families, providers, and agencies, ensuring confidentiality and compliance while delivering respectful, person-centered service.

This role requires at least four years of related experience or education, and may involve driving and occasional travel.

Qualifications

• Bachelor’s degree in social sciences or related field.
• Direct experience working with consumers in social service settings; experience may substitute education.
• Four years of combined education and relevant work experience.

Responsibilities

• Meet with consumers to assess service needs and identify support options.
• Develop individualized care plans per program requirements and consumer needs.
• Coordinate services such as in-home care, transportation, and community resources.
• Monitor and update care plans as circumstances change.
• Complete assessments, documentation, and case records in required systems.
• Collaborate with agency staff, providers, and community partners to support consumers.
• Help consumers address environmental, medical, or social concerns and connect with resources.
• Maintain confidentiality and comply with policies and mandatory reporting.
• Provide respectful, person-centered service to consumers, providers, and partners.
