Job summary

CenterWell Certified Healthcare Corp. is seeking compassionate, organized case managers to serve Wisconsin Family Care members.

You will develop an MCP, coordinate with a Field Care Nurse, and conduct regular assessments and visits to support frail elders and adults with IDD/physical disabilities. Requirements include a Bachelor's in a human services field and 1 year of relevant experience (or 3 years in related work), plus a valid driver's license and 100/300/100 insurance.

Qualifications

• Bachelor's degree in human services or related field required; 1 year experience with frail elders or adults with IDD/physical disabilities, or 3 years in a related field.
• Valid state driver's license and proof of personal vehicle liability insurance (100/300/100).

Responsibilities

• Assess members' health and safety needs to develop a comprehensive Member Care Plan (MCP).
• Coordinate with a Field Care Nurse to address health and safety needs in least restrictive environment.
• Conduct face-to-face social assessments upon enrollment and at least every six months, typically at the member's residence.
• Conduct quarterly in-person visits and maintain monthly phone contact with members.
• Arrange non-medical support services including social integration, community resources, housing, and employment.
• Maintain documentation including case notes, service authorizations, and MCP updates.
