You will provide hands-on support, document care, and assist the manager with daily operations in a fast-paced home and community setting.

Qualifications

High school diploma or GED.
Ability to organize tasks, follow procedures, and ask for help.
Clear, respectful communication with people supported, colleagues, nurses, and partners.
Careful attention to documentation, confidentiality, and follow-through.
Ability to learn and use electronic records and scheduling tools.

Responsibilities

Provide direct support based on each person’s preferences and goals.
Help people participate in activities, appointments, employment programs, and community life.
Manage meals, clothing, transportation, and funds for activities.
Maintain accurate records and follow approved plans.
Coordinate, schedule, and communicate health-related needs.
