Job summary

Access Community Care in Oregon is seeking a Family Direct Support Professional to provide individualized support and skills training to individuals with developmental disabilities, helping them pursue personal outcomes and live as independently as possible.

You will drive to activities, assist with daily living, maintain documentation, and collaborate with a team to ensure safety, dignity, and quality of life.

Qualifications

• Must be 18 years of age or older.
• High school diploma or equivalent.
• Valid driver's license and reliable vehicle.
• Clear background checks as defined by state regulations.
• Ability to lift up to 20 lbs and perform daily physical tasks.
• Strong verbal, reading, and written communication in English.
• Willingness to complete training.
• Experience with developmental disabilities preferred.
• Ability to work well with others and maintain professionalism.
• Flexible hours and changing conditions.
• Certifications may be required by state regulations.

Responsibilities

• Provide supports to individuals by assisting in development and implementation of all support plans and helping achieve personal goals.
• Assist with information gathering for Personal Outcome Measures and other assessments.
• Help maintain a clean, safe, and orderly home by following safety and infection control procedures.
• Assist people with getting to appointments, errands, and various community locations by driving them as needed.
• Maintain and expand knowledge relevant to providing high quality support and services.
• Maintain open communication and cooperate with management, staff, customers, and other support staff.
• Complete proper and timely documentation including daily logs and progress notes.
• Support people with fiscal management and safeguarding their funds.
• Ensure people exercise their rights with dignity and due process.
• Perform other duties as assigned.
