Job summary

Duke Health is seeking a Complex Case Manager (NC LCSW-A or LCSW preferred) to provide psychosocial assessments, coordinate care transitions, and support patients and families during hospitalization. This on-site role emphasizes collaboration with the health care team and community resources.

Qualifications

• Master's degree in social work (MSW) preferred for complex case management roles
• Current licensure as a licensed clinical social worker (LCSW) in NC or eligibility to obtain within 3 years
• Experience in psychosocial assessments, discharge planning, and care transitions

Responsibilities

• Provide psychosocial assessments, diagnosis, and treatment to support patients and families.
• Manage designated caseload for discharge planning and care transitions across the continuum.
• Develop and implement comprehensive psychosocial treatment plans and interventions.
• Coordinate with health care team and community resources for services and support.
• Maintain timely documentation and participate in QA/QI activities.
