Qualifications

• Master's degree in social work preferred for Complex and Transplant CM Positions
• 3 years of relevant experience
• Requires Case Management Certification (ACM or CCM) within 3 years of hire; current LCSW or LCSW-A license in NC

Responsibilities

• Provide psychosocial assessments of patients and families to identify emotional, social, and environmental strengths and problems related to diagnosis and care
• Develop and implement comprehensive psychosocial treatment plans including individual, family, and group therapies as needed
• Coordinate discharge planning and care transitions across the continuum to ensure optimal resource utilization and CMS compliance
• Maintain timely documentation of assessments, discharge arrangements and actions in patient records
• Collaborate with health care team to guide education and support for patients and families
