Job summary

Duke Health Lake Norman Hospital in Mooresville, NC seeks a licensed clinical social worker to manage discharge planning and care transitions for a designated caseload. You will work with a multidisciplinary team to ensure CMS and payer requirements are met while prioritizing patient safety and timely transitions.

Ideal candidates are LCSW or LCSW-A with 3+ years of adult/mental health experience, strong communication, and the ability to work independently in a fast-paced hospital setting.

Qualifications

• Master's degree in social work from an accredited school of social work.
• 3 yrs of relevant adult/mental health experience preferred.
• Current licensure as an LCSW or LCSW-A by the NC Social Work Certification and Licensure Board; notary/Liaison duties may be required.

Responsibilities

• Coordinate discharge planning and care transitions to ensure timely and efficient patient flow.
• Manage a designated caseload to assess, plan, implement and evaluate discharge plans in alignment with CMS and external guidelines.
• Maintain clear, timely documentation and communication with the health care team about discharge planning needs and payer requirements.
• Collaborate with Utilization Management to support negotiations and denial prevention with payers.
• Provide education and guidance to providers, patients and families on benefits, regulations and discharge processes.
