• CCM certification is preferred but not required.

Responsibilities

• Coordinate care for complex hospital patients across the care continuum.
• Lead and collaborate on interdisciplinary discharge planning.
• Educate team members on case management and care coordination concepts.
• Maintain knowledge of program benefits, payer requirements, and community resources.
• Possibly supervise other staff and drive process improvements.
