Job summary

Banner University Medical Center - South Campus in Tucson seeks a Transitional Care Associate to facilitate safe, timely discharge planning and post-acute transitions. The role spans ED referrals and inpatient intake/discharge activities, coordinating with nursing, physicians, and social work teams.

Qualifications

• Must demonstrate effective communication and customer service skills.
• Human relation and time management skills required.
• Bachelor’s degree in social work or related field or equivalent licensure (LPN/Licensed Respiratory Therapist) preferred.

Responsibilities

• Facilitates timely discharge/transfer from hospital to post-acute settings.
• Collaborates with healthcare team to implement care transitions.
• Documents interventions in patient records and discharge plans.
• Coordinates with post-acute providers and community resources.
• Performs follow-up calls to patients and caregivers as needed.
• Communicates discharge dates and care settings to patients and families.
• Maintains knowledge of Medicare/Medicaid benefits for transition planning.
• Adheres to established procedures under supervision for discharge planning.
