Banner Wyoming Medical Center is seeking a Case Manager in Care Coordination to provide comprehensive care coordination for patients as assigned. The role oversees discharge planning, coordinates with physicians, payers and the care team to ensure safe transitions and timely care.

The position supports the interdisciplinary team across units, conducts initial assessments, participates in rounds, and communicates discharge plans to patients and families.

Formación

• RN: bachelor's degree in case management or health care with active RN license.
• Social Worker: MSW and LMSW/LCSW (or equivalent) with license within 6 months of hire.
• BLS certification required for acute care assignments.
• Requires ~2 years clinical experience and strong critical thinking, problem-solving, communication and time management skills.

Responsabilidades

• Manage patients across the health care continuum to optimize clinical, financial, operational and satisfaction outcomes.
• Lead process improvement activities for patient populations to improve outcomes.
• Collaboratively develop and manage the interdisciplinary discharge plan; communicate it across the care continuum.
• Educate team on case management concepts and integrate into daily practice.
• May supervise other staff and coordinate with physicians, payers and regulatory bodies.
• Provide discharge planning support and ensure timely transitions for patients.
