Job summary

A Bright Future, Inc. is seeking a Community Support Facilitator (CSF) to lead and coordinate Independent Living Services (ILS) and Supported Living Services (SLS). This role supervises direct staff, collaborates with families, and ensures person-centered plans are delivered with dignity in community settings.

You will work closely with the Program Director, travel between participant homes and program locations, and participate in on-call rotation to support outcomes and safety.

Qualifications

• Bachelor's degree preferred in Psychology, Human Services, Social Work, Special Education, or related field.
• Four (4) or more years of relevant experience may be considered in lieu of a degree.
• Minimum of two (2) years of experience with individuals with developmental or intellectual disabilities.
• Experience supporting individuals with behavioral, medical, psychiatric, or other complex needs preferred.
• Experience working with California Regional Centers and/or ILS/SLS programs preferred.

Responsibilities

• Provide leadership, coordination, and oversight of services for individuals receiving ILS and SLS.
• Supervise and support direct service staff, coordinate individualized services, ensure IPP/ISP alignment.
• Coordinate services with families, staff, Regional Centers, and service providers.
• Monitor implementation of participant goals and adjust support as needed.
• Maintain accurate participant documentation and ensure regulatory compliance.
• Monitor staff performance, provide coaching, and maintain adequate staffing levels.
• Lead quality assurance activities and support regional center audits.

Education

Bachelor's degree in Psychology, Human Services, Social Work, Special Education, or a related field preferred
