Job summary

Medicalincs LLC is seeking a Community Health Worker to support behavioral health and chronic disease prevention across PG County and remote sites. This role links individuals, families, schools, providers, and community resources to promote access to care and health equity.

In a people-centered team, you will engage diverse communities, provide education, navigate services, and assist with program outreach, advocacy, and capacity-building activities.

Qualifications

• Bachelor's degree preferred; 1-3 years in community health settings is typical.
• Bilingual in English and Spanish preferred.
• Strong communication, teamwork and organizational skills are essential.

Responsibilities

• Increase utilization of preventive care and mental health services.
• Assess unmet social needs of clients/patients.
• Develop intervention plans with clients and families under supervision.
• Educate on community resources and assist with access to services.
• Complete school and community visits to improve engagement.
• Monitor progress and adjust plans as needed.
• Attend daily/weekly care meetings and document activities.
• Share concerns with the care team during check-ins.
• Provide feedback for program improvements.
• Flexibility to work occasional evenings and weekends.
