Job summary

• High School diploma or equivalent required.
• Valid Driver’s License required.
• Experience with patient navigation and community health programs preferred.

Responsibilities

• Conducts in-home, clinic-based, telephonic, and community-based wellness visits.
• Completes health risk assessments and follow-up to identify barriers to care.
• Provides support, education, and reinforcement to help patients understand and follow care plans.
• Supports medication adherence by reminders and escalation to the clinical team as needed.
• Assists with appointment reminders, follow-up care, and preventive screenings.
• Documents encounters and follow-up activities in the electronic system.
