Job summary

The CHRISTUS Health Community Health Worker in Corpus Christi, TX conducts advanced community outreach and coordinates referrals to local health and social services. Qualifications

• Knowledge of community resources and how to refer patients to them.
• Bachelor’s Degree preferred.

Responsibilities

• Provides culturally appropriate support and information for patients and families during structured visits and home visits.
• Refers patients to community resources.
• Helps patients understand how to use healthcare and social services and manage their Patient Health Record (PHR).
• Maintains a holistic and interdisciplinary approach to care management and health promotion.
• Builds relationships with patients, families, healthcare providers and the community.
• Assists patients in finding and using Medical Home, pharmacy assistance programs, dental services, diagnostic services, transportation, behavioral health, and other services.
• Follows up on referrals and coordinates care through care pathways.
• Documents activities according to organizational policies and procedures; maintains patient registry.
