Responsibilities

• Provide culturally appropriate support and information for patients and families during structured visits and follow-up calls.
• Know resources in the community and refer patients to them.
• Assist patients in understanding how to access healthcare and social services and manage their health records.
• Adopt a holistic, interdisciplinary approach to care management and health promotion.
• Build relationships with patients, families, healthcare providers and the community.
• Assist patients in finding and using a Medical Home, pharmacy assistance, dental services, diagnostics, transportation, and behavioral health services.
• Refer patients to healthcare and social services and follow up on referrals.
• Follow evidence-based care pathways to address health and social concerns.
• Document activities and maintain patient registries per policy.
• Coordinate with supervisors to schedule and address healthcare needs.
• Educate patients and families in healthy lifestyles and promote behavior change.
• Encourage patients to set goals, identify barriers, and monitor their conditions.
