Job summary

Adventist Health will be seeking a Community Health Worker to assist patients and families in navigating local services and resources. The role includes conducting intake interviews, providing health education, and supporting enrollment for uninsured patients under supervision.

The ideal candidate will have a high school education (GED preferred) and may hold an associate/technical degree with formal CHW training preferred. Willits, CA location with full-time hours.

Qualifications

• High School Education/GED or equivalent preferred.
• Associate’s/Technical Degree or equivalent combination of education/related experience preferred.
• Successful completion of a Community Health Worker training program preferred.

Responsibilities

• Provides interpretation and translation services and health education, offering general support to patients and families.
• Performs intake interviews and enrolls/refers patients into local services; follows up with patients by phone.
• Helps patients complete applications and registrations; determines eligibility for uninsured patients and assists with enrollment.
• Supports transportation needs and maintains communication with patients, families and staff.
