Qualifications

Responsibilities

• Mentor Community Health Workers and provide oversight in member care.
• Coordinate care and referrals to improve health outcomes.
• Serve as primary contact and conduct in-home visits and health education.
• Enter client data into software daily and document case notes.
• Advocate for members and collaborate with the care team as needed.
• Support members in developing health management plans and access to services.
