Optum is seeking a Senior Advanced Practice Clinician (NP/PA) in Lane County, OR to deliver longitudinal, value-based care in skilled nursing facilities. You’ll manage your own daily schedule with no production quotas or on-call requirements, emphasizing high-quality patient relationships.

Role supports independence, strong team collaboration, and meaningful patient outcomes, with competitive base pay, bonuses, and relocation assistance. Field-based, full-time in Lane County.

Qualifications

• Graduate of an accredited MSN or DNP program.
• Active and unrestricted license in the state of residence (NP) or eligible by start date.
• Board certification through AANP or ANCC (NP) or NCCPA (PA).

Responsabilités

• Conduct comprehensive patient assessments.
• Manage acute and chronic conditions with treatment plans.
• Provide care during short-term sub-acute rehabilitation episodes.
• Lead goals-of-care and advance care planning discussions.
• Collaborate with PCPs, facility teams, and interdisciplinary partners.
• Educate patients and families on treatment plans and disease management.
• Maintain accurate and timely clinical documentation.
