Qualifications
- MSN or DNP from an accredited program.
- Active NP license or ability to obtain by start date.
- AANP or ANCC certification in an approved NP specialty.
- Graduate of an accredited PA program with an active PA license.
- NCCPA certification for PA candidates.
Responsibilities
- Conduct comprehensive patient assessments.
- Manage acute and chronic conditions, diagnostics, and treatment plans.
- Provide care during sub-acute rehabilitation episodes.
- Lead goals-of-care and advance care planning discussions.
- Collaborate with caregivers, facility staff, PCPs, and interdisciplinary teams.
- Educate patients and caregivers.
- Maintain accurate clinical documentation.
