Job summary

CJE SeniorLife is seeking a Cognitive Care Navigator to support older adults with memory concerns and their families. You will coordinate cognitive screening, provide tailored consultations, and connect individuals to CJE programs and community resources.

This on-site role emphasizes early identification, care planning, and supportive guidance within a multidisciplinary team. The position requires an MSW and at least 3 years serving older adults, with a focus on caregiver support and resource

Qualifications

• Requires a master’s degree in social work (MSW).
• Minimum 3 years experience with older adults, caregivers, or dementia-related conditions.
• Experience in counseling, care coordination, navigation, caregiver support, assessments, and resource referral is required.
• Knowledge of ADRD and community resources is preferred.

Responsibilities

• Provide cognitive screening consultations, education, support, and referrals.
• Coordinate memory screening programs and related documentation.
• Support cognitive health outreach and early intervention efforts in the community.
• Deliver individualized cognitive care consultations and educate families about diagnoses and options.
• Assist with care planning, transitions, safety concerns, and ongoing guidance.
