Job summary

Dot Cares LLC is seeking qualified individuals with a passion for working with older adults. Experience in Social Work or Registered Nurse is preferred. You will assess clients in their environment, develop individual care plans with families and care teams, and coordinate with physicians and legal teams.

Qualifications

• Master’s degree in social work.
• Minimum one year of experience in health care and social work.
• Valid driver’s license.
• Psych experience is a plus.

Responsibilities

• Quarterback the patient's needs to make sure they are met.
• Participate in patients’ plans of care. Develop "To Do" list outlining priorities for health and safety issues
• Assess the psychosocial status of the patients as related to their illness
• Schedule regular visits to oversee plans of care and those providing services such as home health aides.
• Plan interventions based on patient's needs and findings
• Maintain accurate and up-to-date records
