Job summary

Advocate Health is seeking a Licensed Clinical Social Worker (LCSW) for the Memory Counseling Program within Gerontology & Geriatric Medicine. The role focuses on behavioral health services, case management, care coordination, and caregiver support for cognitive impairment and dementia.

The clinician will conduct psychosocial assessments, develop treatment plans, facilitate family meetings, and connect families with community resources in an interdisciplinary team setting.

Qualifications

• Master of Social Work required.
• Clinical social work experience required.
• Minimum 3 years' mental health experience.
• Experience with older adults or dementia preferred.

Responsibilities

• Conduct psychosocial assessments and mental health evaluations.
• Provide individual psychotherapy and supportive counseling for dementia caregivers.
• Perform case management and care coordination activities, including GUIDE Program visits.
• Develop and implement psychosocial treatment plans.
• Facilitate family meetings and caregiver support groups.
• Connect patients and families with behavioral health and community resources.
• Collaborate with physicians, nurses, and other professionals to support patient care.
• Document services and interventions in the EMR.
• Assist with program outcome tracking and quality improvement.
