Job summary
CJE SeniorLife is seeking a Cognitive Care Navigator to support older adults with memory concerns and their families. You will coordinate cognitive screening, provide tailored consultations, and connect individuals to CJE programs and community resources.

Qualifications

As a Cognitive Care Navigator, you will support individuals experiencing memory loss, cognitive changes, dementia, and related conditions, along with their care partners and families. You will help them understand their options, access appropriate resources, and feel supported before, during, and after diagnosis.
You will provide cognitive care navigation, coordinate memory-screening initiatives, support caregivers, and contribute to community education and outreach. Working with Adult Day Services, Clinical Intake, Care Management, and other interdisciplinary teams, you will help promote early identification and coordinated support for individuals and families navigating cognitive change.
- Compensation: $60,000 to $65,000.
- Employment status: Full‑time, salaried, exempt.
- Schedule: Monday to Friday, 8:30 a.m. to 5:00 p.m.
- Location: Zoller/Kaplan Families Center for Cognitive Services.
- Work arrangement: On‑site.
- Travel: None.
- Union status: Union membership with SEIU Local 73 is an option.

- Provide consultations, education, support, and referrals to individuals and care partners experiencing cognitive concerns.
- Coordinate the Alzheimer's Foundation of America (AFA) National Memory Screening Program and related screening initiatives.
- Maintain screening documentation, outcomes tracking, and program reporting.
- Support cognitive health awareness, outreach, and early intervention efforts throughout the community.
- Provide individualized cognitive care consultations for individuals experiencing memory concerns and cognitive changes and their care partners.
- Educate and support individuals and families in understanding diagnoses, disease progression, care options, and available resources.
- Connect individuals and care partners with appropriate CJE programs, community services, and support.
- Assist with care planning, transitions, safety concerns, and other cognitive care needs.
- Offer ongoing guidance during periods of adjustment and decision‑making.
- Provide short‑term supportive counseling, emotional support, and problem‑solving assistance to individuals and caregivers.
- Facilitate caregiver education, support, and learning opportunities and programs.
- Develop and maintain caregiver resources and educational materials.
- Conduct presentations, workshops, and educational programs on cognitive health, dementia, caregiving, and related topics.
- Support outreach efforts that increase awareness of CJE’s cognitive services and available community resources.
- Assist with program development, implementation, evaluation, and growth initiatives within the Center for Cognitive Services.
- Support grant activities, data collection, outcomes tracking, and reporting requirements.
- Collaborate with Adult Day Services, Care Management, Clinical Intake, and other CJE departments to ensure coordinated support for individuals and care partners.
- Serve as an internal resource on dementia, cognitive impairment, caregiver support, and community resources.
- Maintain accurate documentation in accordance with agency standards, regulatory requirements, and HIPAA guidelines.
Required qualifications
- A master’s degree in social work (MSW) is required.
- A minimum of 3 years of experience with older adults, caregivers, or individuals with dementia or related conditions is required.
- Experience in counseling, care coordination, navigation, caregiver support, assessments, and resource referral is required.
- Knowledge of ADRD, mild neurocognitive disorder, caregiver needs, and community resources is required.
Preferred qualifications
- Having or being eligible for an Illinois Licensed Clinical Social Worker (LCSW) licensure is preferred.
- Experience with outreach, public speaking, and program development preferred.
Cognitive changes can leave individuals and families uncertain about what comes next. In this role, you will help them better understand their options, connect with appropriate support, and make informed decisions throughout the course of cognitive change. Your work will also strengthen early identification, caregiver support, and coordination across CJE's cognitive care continuum, helping individuals and their care partners feel supported, informed, and connected during challenging periods.
