Providence Centralia Hospital is seeking a Senior Clinical Pharmacist to collaborate with physicians and staff to deliver comprehensive, high-quality care. You will support patient care and population management using disease registries and care plans.

Qualifications

• Bachelor's Degree in Pharmacy from an accredited School of Pharmacy or
• PharmD in Pharmacy from an accredited School of Pharmacy.
• Postgraduate Year One (PGY-1) Pharmacy Residency or equivalent education/experience.
• Washington Pharmacist License upon hire.
• Certification from Board of Pharmacy Specialties within 2 years of hire.
• Board of Pharmacy Preceptor License upon hire when eligible and applicable.

Responsabilités

• Provide collaborative practice services in collaboration with the healthcare team.
• Utilize registries for chronic diseases and reports/tools to identify target populations and improve therapeutic outcomes through education and therapy optimization.
