Qualifications
- Bachelor's Degree in Pharmacy from an accredited School of Pharmacy or PharmD in Pharmacy from an accredited School of Pharmacy.
- Postgraduate Year One (PGY-1) Pharmacy Residency or equivalent education/experience.
- Washington Pharmacist License upon hire.
- Board of Pharmacy Specialties certification within 2 years of hire.
- Board of Pharmacy Preceptor License upon hire where applicable.
- 3 years pharmacy practice with at least 2 years related experience in lieu of PGY-1.
Responsibilities
- Provide collaborative practice services with the healthcare team.
- Utilize registries and tools to identify target populations and enhance therapeutic outcomes through education and drug therapy optimization.
