Job summary
Providence Centralia Hospital is seeking a Senior Clinical Pharmacist to collaborate with physicians and staff to deliver coordinated, high-quality patient care. You will participate in collaborative practice and utilize tools to optimize drug therapy and educate patients.

Qualifications
- Bachelor's Degree in Pharmacy from an accredited School of Pharmacy.
- PharmD in Pharmacy from an accredited School of Pharmacy.
- Postgraduate Year One (PGY-1) Pharmacy Residency or equivalent education/experience.
- Upon hire: Washington Pharmacist License.
- Within 2 years of hire: Certification from Board of Pharmacy Specialties.
- Upon hire: Board of Pharmacy Preceptor License - when eligible and where applicable.
- 3 years Pharmacy practice experience with at least 2 years of related experience in lieu of PGY-1 Pharmacy Residency.
Responsibilities
- (1) Patient Care - The Senior Clinical Pharmacist is responsible for providing collaborative practice services in collaboration with the healthcare team.
- (2) Population Management - The Senior Clinical Pharmacist may utilize registries for chronic diseases and reports/tools to identify target populations/patients and enhance therapeutic outcomes through disease/medication education and drug therapy optimization.
