Qualifications
- Bachelor's Degree in PHARMACY from an accredited School of Pharmacy.
- PharmD in PHARMACY from an accredited School of Pharmacy.
- Coursework/Training: Postgraduate Year One (PGY-1) Pharmacy Residency or equivalent education/experience.
- Upon hire: Washington Pharmacist License.
- Within 2 years of hire: Certification from Board of Pharmacy Specialties.
- Upon hire: Board of Pharmacy Preceptor License - when eligible and where applicable.
- 3 years Pharmacy practice experience with at least 2 years of related experience in lieu of Postgraduate Year One (PGY-1) Pharmacy Residency.
Responsibilities
- Provide collaborative practice services in collaboration with the healthcare team.
- Population management: use registries and reports/tools to identify target populations and optimize therapy.
- Deliver patient-focused care at Providence Centralia Hospital and contribute to the team’s mission.
