Job summary

West Side Federation for Senior and Supportive Housing is seeking a Clinical Coordinator II to supervise professional social work services, including supervision of social workers, case managers, recreation, medication and other program staff, while managing a small caseload.

The role reports to the Managing Director or Director of Clinical and Social Services and collaborates with the Building Manager to ensure quality care and regulatory compliance across the site.

Qualifications

• Minimum of 3 years supervisory experience required.
• Minimum of 5+ years working with older adults or people with history of homelessness, chronic mental health issues or substance abuse.
• Proven ability to build, foster and lead collaborative groups and workshops for social service staff between various programs.

Responsibilities

• Provide clinical and case management supervision to all social work and case management staff.
• Provide access to clinical supervision to all staff as needed.
• As a member of the Intake Committee, review all applications for admission and participate in home visits and admission decisions.
• Provide clinical support and resources for psychiatric emergencies.
• Review and sign support plans, case management summaries, case notes and psychosocial histories.
• Ensure compliance with contract and regulatory requirements through regular chart audits.
