Job summary

Meta Care Inc. is seeking a part-time Care Manager for Senior Clergy to guide and advocate for diocesan leaders in navigating healthcare, benefits, and community resources.

This role focuses on coordination, not hands-on clinical care, and aims to promote wellness and seamless access to services. You will build trusted relationships through visits and outreach, coordinate appointments, support hospital transitions, and work with a multidisciplinary team to tailor assistance.

Qualifications

• Minimum 3 years of healthcare experience required.
• RN, LPN, CNA or MA preferred; active license not required.
• Experience in clinical or medical office settings.
• Excellent communication and listening skills.
• Strong organizational abilities and attention to detail.

Responsibilities

• Build trusted relationships with senior clergy through outreach and visits.
• Coordinate medical, dental, and specialist appointments.
• Navigate healthcare systems and provider networks for members.
• Maintain accurate documentation and protect PHI/ HIPAA.
• Travel to member locations and participate in client meetings.
