Meta Care Inc. is seeking a Care Manager to serve clergy of the Catholic Archdiocese of Galveston-Houston, coordinating health services and advocating for patient-centered care.

You will support navigation of benefits, coordinate appointments, and promote wellness through relationship-based outreach. The role emphasizes compassionate coordination rather than direct clinical care, with opportunities to collaborate across care teams and community resources.

Qualifications

• Minimum 3 years of healthcare experience.
• RN or BSN (active license not required).
• Experience navigating healthcare systems and benefits.
• Strong communication and listening skills.
• Excellent organizational abilities and attention to detail.
• Ability to work independently with team collaboration.

Responsabilités

• Build trusted relationships with clergy through regular outreach and visits.
• Coordinate medical, dental, and specialist appointments to ensure seamless care.
• Advocate for members during hospitalizations and provide follow-up coordination.
• Collaborate with Meta Care’s support team including pharmacists, nutritionists, and social workers.
• Maintain accurate documentation and strict confidentiality of PHI and HIPAA guidelines.
