Job summary

Meta Care Inc. is seeking a Care Manager to advocate for eligible clergy in the Catholic Diocese of Rockford, IL. This non-clinical role emphasizes care navigation, education, and resource coordination without providing medical treatment.

You will build relationships, coordinate appointments, and connect members with community resources while adhering to privacy and HIPAA standards. This is a full-time position in a mission-driven organization serving priests.

Qualifications

• Minimum 3 years of healthcare experience.
• Preferred LPN or RN (active license not required).
• Bi-lingual preferred (Spanish).
• Experience in clinical or medical office settings with advocacy and care coordination.
• Proficiency in Microsoft Office (Word, Excel, PowerPoint).

Responsibilities

• Build trusted relationships with eligible priests through regular outreach and personal visits.
• Serve as a primary contact for clergy needing assistance navigating healthcare and benefits.
• Coordinate medical, dental, and specialist appointments for seamless care.
• Assist members in navigating healthcare systems and provider networks.
• Coordinate benefit coordination and collaborate with plan design vendors.
• Provide post-hospitalization follow-up and non-clinical care coordination.
• Engage in on-site visits to support service coordination and safety.
