Job summary

The Catholic Diocese of Tyler is seeking a Care Manager to support clergy with navigation of healthcare and benefits, acting as an advocate and coordinator. This role emphasizes compassionate service and collaboration with Meta Care’s team to ensure members receive the right care at the right time.

You will build relationships through visits and outreach, document interactions, and maintain confidentiality while aiding transitions from hospital to home or facility-based care.

Qualifications

• Minimum 3 years of healthcare experience.
• Preferred LPN or RN (active license not required).
• Experience in clinical or medical office settings with a passion for helping individuals navigate the healthcare system.

Responsibilities

• Build trusted relationships with clergy through regular outreach and personal visits.
• Coordinate medical, dental, and specialist appointments to ensure seamless care experiences.
• Assist members in navigating healthcare systems and provider networks.
• Provide post-hospitalization follow-up and care coordination.
• Maintain accurate documentation and confidentiality in accordance with PHI and HIPAA.
