Job summary

COPE Health Solutions is seeking a Community Health Worker II to support the Enhanced Care Management program in Los Angeles-area communities. The CHW II will work primarily in person, engaging homeless and other high-need members, coordinating care, and promoting transportation and housing access.

The role includes mentoring junior CHWs, maintaining documentation, and collaborating with the Program Manager to strengthen ECM workflows.

Qualifications

• Minimum two years of CHW or similar role experience.
• Experience in street and community outreach preferred.
• Experience with ECM, Whole Person Care, Health Homes, or Medi-Cal care management preferred.
• Spanish language fluency preferred.
• Valid California Driver’s License and insured vehicle.

Responsibilities

• Identify, locate, and engage ECM-eligible members through outreach.
• Explain ECM services and enroll members; liaison between members and services.
• Build referral relationships with shelters, clinics, hospitals, and community orgs.
• Meet members for assessments and create individual care plans.
• Coordinate appointments, transportation, and follow-up across care settings.
• Provide health education and support self-management.
• Document interactions in EMR within 24 hours.
